Release of Liability Waiver & Photo Release
READ CAREFULLY-THIS AFFECTS YOUR LEGAL RIGHTS
In exchange for participation in the activity of “Tiny Kicks Academy” organized by Max Weinstein (“Coach Max”), of 404 Hermosa Ave Unit 1, Hermosa Beach, CA 90254 and/or use of the property, facilities and services that Coach Max secures for “Tiny Kicks Academy”, I agree for myself and for the members of my family, to the following:
I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Coach Max, or the employees, representatives or agents of Coach Max. If my child refuses to play or follow directions by Coach Max, or the employees, they reserve the right to remove us from the program and will issue a refund for any remaining sessions that were previously paid for.
I recognize that there are certain inherent risks associated with the above described activity, “Tiny Kicks Academy”, and I assume full responsibility for personal injury to myself and my family members, and further release and discharge Coach Max for injury, loss or damage arising out of my or my family’s use of or presence upon the facilities secured by Coach Max, whether caused by the fault of myself, my family, Coach Max or other third parties.
I agree to indemnify and defend Coach Max against all claims, cause of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family’s use of or presence upon the facilities while under the instruction of Coach Max.
I consent to the participation of my child in “Tiny Kicks Academy”, and agree on behalf of the above minor to all of the terms and conditions of this Agreement. By signing this Release of Liability, I represent that I have legal authority over and custody of the child attending the soccer training session.
In the event of an injury to the above minor during the above described activities, if I am not present, I give my permission to Coach Max or to the employees, representatives or agents of Coach Max to arrange for necessary medical treatment for which I shall be financially responsible. This temporary authority will remain in effect during any time that I am not present and the above minor is attending “Tiny Kicks Academy”, while under Coach Max’s or the employees, representatives or agents of Coach Max’s instruction, and they shall have the following powers:
The power to seek appropriate medical treatment or attention on behalf of my child as may be required by the circumstances, including without limitation, that of a medical physician and/or a hospital;
The power to authorize medical treatment or medical procedures in an emergency situation; and
The power to make appropriate decisions regarding clothing, bodily nourishment and shelter.
Any legal or equitable claim that may arise from participation in the above shall be resolved through arbitration, under California law.
Photo Release: I, hereby grant Tiny Kicks Academy permission to use my child's likeness in a photograph or video in any and all of its publications, including but not limited to all of Tiny Kicks Academy, LLC's printed and digital publications.
I will receive no financial compensation. I hereby irrevocably authorize Tiny Kicks Academy, LLC to edit, alter, copy, exhibit, publish or distribute the videos or photos for purposes of publicizing Tiny Kicks Academy, LLC's programs or for any other related, lawful purpose.
I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my child's likeness appears.
I waive any right to royalties or other compensation arising out of, or related to, the use of the photograph.
I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY ACCEPTING THIS RELEASE OF LIABILITY WAIVER, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS.

For good and valuable consideration, the receipt of which is hereby acknowledged, I, hereby grant Tiny Kicks Academy permission to use my child's likeness in a photograph or video in any and all of its publications, including but not limited to all of Tiny Kicks Academy, LLC's printed and digital publications. A copy of the videos and photographs will be shared with me after the session.
I acknowledge that since my participation with Tiny Kicks Academy, LLC is voluntary, I will receive no financial compensation. I hereby irrevocably authorize Tiny Kicks Academy, LLC to edit, alter, copy, exhibit, publish or distribute
This photo for purposes of publicizing Tiny Kicks Academy, LLC's programs or for any other related, lawful purpose.
